When the head moves, the peripheral vestibular organs must respond. Because the optokinetic system and the vestibulo-ocular reflex (VOR) share the same neural circuitry via the three-neuron arc, we push this massive visual signal directly into the vestibular nuclei, actively charging the velocity storage integrator and demanding that the peripheral reflexes readapt.

The Simultaneous Action of Linear and Angular Integration

Because the patient presents with a multidirectional sensation of motion, we systematically challenge the eigenvectors of all three geometric planes. Crucially, a subtle physical turn or tilt of the head inherently induces both rotation and linear translation. Stop for a moment and understand vection. For instance, if you look from left to right in the upper right, you can see the stripes move the eyes and produce a left-beating nystagmus. This is absolute torque and can lead to the angular (Horizontal Canal) and linear (Utricle) VOR to fire.

The Washout Phase: The Critical Window for Surface Dependency

We apply these multi-planar visual conflicts to deliberately overload the maladaptive visual strategy. However, the brain's actual recalibration occurs the moment the screen goes blank.

During the 30-second washout phase, the velocity storage mechanism aggressively discharges. This is the exact window of maximum neuroplastic vulnerability.

At this precise moment, we must control the patient's position through aggressive somatosensory and vestibular anchoring. The patient must actively engage their true surface dependency. Whether they are sitting rigidly or lying flat on a treatment table, they must drive their joint capsules, muscle spindles, Golgi tendon organs, and pacinian corpuscles into the supporting surface.

Balance is inherently a subcortical process. We do not consciously think about our otoliths firing when we sit in a chair. The constant, hypervigilant perception of motion in MdDS patients represents a systemic failure that has breached the cortical level, resulting in the massive behavioral overlay and anxiety we see in the clinic.

If an MdDS patient could simply accept the baseline vestibular reference—the undeniable proof that they are mechanically stable—they would recover. The treatment must force the patient to accept this vestibular reality.

By applying a precise, intense, multi-planar visual conflict that systematically triggers the linear and angular VOR, followed immediately by aggressive active surface anchoring, we exhaust the visual dependency.
